For most of its life, orexin was a beautiful piece of basic science and a difficult commercial subject. In 2026 it became a priced asset class. Within a few months, Eli Lilly closed its acquisition of Centessa Pharmaceuticals and its orexin agonist pipeline; the first orexin receptor agonist was approved in three major markets in the space of five weeks; and in September, the Lasker Foundation gave its Albert Lasker Basic Medical Research Award to the two scientists who, a generation earlier, had identified orexin and shown that its absence causes narcolepsy. Anyone who still files sleep under minor neuroscience categories should re-read those three facts together.
It is worth being precise about what the much-discussed link between orexin and GLP-1 is, and what it is not. Orexin is a hypothalamic neuropeptide system that stabilizes wakefulness and also participates in feeding, reward, and energy expenditure. The GLP-1 system intersects with it anatomically: in work dating to 2004, GLP-1 was shown to excite orexin neurons, and orexin neurons in turn project to brainstem regions where GLP-1 is produced. Nearly all of that evidence is preclinical. No study has shown that approved GLP-1 medicines work through orexin in patients, no combination of the two mechanisms is in clinical development, and the suggestive mouse work linking enhanced orexin signaling to resistance to diet-induced obesity has produced no clinical obesity program that we can find. The convergence is real, but at present it is a convergence of indications and of portfolios, not of molecules.
三款双食欲素受体拮抗剂 suvorexant(2014)、lemborexant(2019)和 daridorexant(2022)都已拿到 FDA 的失眠批准,十年的使用确立了这个靶点可成药、这一类药经得起时间。 Blocking orexin came first. Three dual orexin receptor antagonists — suvorexant (2014), lemborexant (2019), and daridorexant (2022) — hold FDA approvals for insomnia, and a decade of use has established the target as druggable and the class as durable. The interesting question now is how far antagonism travels beyond insomnia. Johnson & Johnson's seltorexant, a selective orexin-2 receptor antagonist, was tested as an adjunctive treatment for major depressive disorder with insomnia symptoms. In the Phase 3 MDD3005 study reported in September 2025, response rates were numerically higher than with quetiapine extended-release (57.4 percent versus 53.4 percent), but the primary endpoint did not reach statistical significance; tolerability favored seltorexant, including less somnolence and less weight gain (an average of 0.5 kg versus 2.1 kg). That is the honest shape of expansion in this field: the pharmacology travels, and the endpoints decide.
Activating orexin is the harder and more consequential direction, because narcolepsy type 1 is a deficiency disease: the brain has lost the orexin-producing neurons themselves. Takeda's oveporexton, an oral orexin receptor 2 agonist, met every primary and secondary endpoint in two global Phase 3 studies, with p-values below 0.001 across all doses at week 12. Regulators then moved in quick succession: approvals in China on July 22, 2026, in the United States on August 5, 2026, and in Japan within the same five-week span. The FDA described oveporexton as the first medicine approved to treat narcolepsy type 1 as a complete disorder, and the first to work by directly targeting the loss of orexin signaling that causes it. Approval, however, is not availability: launches outside the United States began in October, while the U.S. launch is set for November 2026, following completion of Drug Enforcement Administration scheduling.
The first mover will not be alone for long. Alkermes has reported positive Phase 2 results for alixorexton in both narcolepsy type 1 and type 2, initiated its global Phase 3 Brilliance program in April 2026, and seen the type 1 data published in The Lancet Neurology. Behind it sits Lilly's newly acquired cleminorexton, with Phase 2a data across narcolepsy type 1, narcolepsy type 2, and idiopathic hypersomnia. In this field, a pioneer's advantage is likely to be measured in readout intervals rather than in years.
GLP-1, meanwhile, has entered sleep from the metabolic side, by a completely different route. On December 20, 2024, the FDA approved tirzepatide for moderate to severe obstructive sleep apnea in adults with obesity, the first medication ever approved for that condition. The agency's own explanation was weight-mediated: the drug activates receptors for GLP-1 and GIP, reduces appetite and body weight, and, by reducing body weight, improves sleep apnea. Precision matters here, because the class is often discussed as if the label were shared. As of this writing, only one molecule in the class holds a sleep indication; semaglutide does not. A convergence narrative that ignores that distinction will not survive a careful reader.
The portfolio convergence is the part of the story that belongs to dealmakers. On March 31, 2026, Lilly — the company that owns the defining GLP-1/GIP franchise — announced that it would acquire Centessa for $38.00 per share in cash plus a contingent value right worth up to $9.00 per share: an upfront equity value of approximately $6.3 billion, with up to approximately $1.5 billion more if the contingent milestones are met. The transaction closed on June 24, 2026. Centessa's orexin agonist series, led by cleminorexton, was co-discovered with Nxera Pharma of Japan, which retains milestone and royalty rights in the programs. The value chain, in other words, was cross-border before it was Lilly's. What the deal says is simple: the strongest metabolic franchise in the industry decided that sleep-wake biology belonged in the same portfolio.
The New England Journal of Medicine Three things deserve attention from anyone pricing orexin assets now. First, the class carries a documented liver lesson: Takeda suspended dosing of its earlier agonist, TAK-994, in 2021 after a liver toxicity signal emerged; the program was discontinued, and the study's eventual publication in The New England Journal of Medicine in 2023 included cases meeting Hy's law criteria for drug-induced liver injury. Liver chemistry is not a footnote in orexin diligence; it is the first page. Second, scheduling is economics: the gap between oveporexton's August approval and its November 2026 U.S. launch is a reminder that controlled-substance status belongs in valuation models, not only in regulatory plans. Third, expansion will be decided by readouts: whether orexin agonism extends convincingly from narcolepsy type 1 to type 2 and idiopathic hypersomnia, and whether the GLP-1 class advances in sleep through dedicated trials rather than through the spillover of weight loss.
Megs was founded in 2010 and has worked on cross-border transactions since 2016; the work described by our headline — Connecting Project Sponsors and Investors Between China and the U.S. — increasingly involves fields, like this one, where the science, the assets, and the counterparties sit in different systems. Sleep and metabolism were separate conversations for a generation. The events of 2026 suggest they will not remain so. Our role remains a modest one: to help each side understand what the other side is actually looking at.
